Treatment request

Online messages to our consultation and treatment team

Dear patients and referring colleagues,

Thank you for your request to our Pancreas Center. We will organize all appointments needed for your treatment as quickly as possible. You can support us in this by sending us documents in advance. If you are contacting the PANKREAS CENTER for the first time or are first presentation of a patient we would like to ask you to use the data tele-upload and to fill out the data request as completely as possible, while for repeated presentations or follow-up checks without document upload, please use the small compact form is sufficient for an appointment request.

Here is a list of the data we require or additionally request:

• Copies of findings, doctor's letters, lab results (please include current tumor markers CEA and CA 19.9 if possible).
• An original CD with the current imaging (CT or MRT/ MRCP, if possible max. 6 weeks old) or alternatively please the QR code or the access data to these examinations, which were given to you by the radiological colleagues.
• A referral slip from your general practitioner
• A short informal letter with your request and description of your complaints

In order to assess whether we can provide a reasonable treatment option and to create an individualized treatment plan, our team needs some information and medical data.
Please fill out the form as completely as possible with the record of medical data essential to the assessment in our data tele-upload .

For ongoing treatment or aftercare

To begin planning further treatment and provide an appointment, please describe your current concerns to us online using the highly abbreviated compact form. If, in addition to a written message, you need to send us any new findings, reports or a copy of the referral slip, please use the data-secure Tele-Upload in our teleportal.

TO THE COMPACT FORM

 

Your records and especially the imaging will be reviewed by our physicians in the interdisciplinary team (surgeons, internists and radiologists) and the further treatment concept will be discussed. In urgent cases, e.g. pancreatic carcinoma or suspected carcinoma, you will receive an appointment at our European Pancreas Center within the next few days. Of course, in these urgent cases that cannot be postponed, prompt surgery appointments will also be scheduled.
